The flag's ratio is 2:3. It is a blue over white over blue horizontal triband with ratio 1:3:1. The coat of arms is in the centre of the white stripe.

The shield is divided per fess. Above in a silver (= white) field is a bearded head in natural colour. Below in a blue field are three silver (= white) fish. Above the shield is a helmet crested by five white plumes and having blue and white scarves.
Meaning:
According to source Barth grew around a castle of the Princes of Rügen and was built around 1200. The city was first mentioned in 1232. In the late 13th century Barth gained city rights. The beard (German: Bart) is canting. The head already appeared on a seal from 1404. The meaning or date of addition of the three fish is not known. The crest had been added in 1992.
