I recently came across the flag of the Office of the First Lady of the Dominican Republic (Despacho Primera Dama), the First Lady, of course, being the wife of the sitting President of the Republic. Apparently, judging from the photographs I was able to find online, it looks like both the seal of the Despacho Primera Dama and the flag may change with successive Presidential administrations and First Ladies.

The current flag used by the Despacho Primera Dama is white with the current seal in the center. The seal, which I assume was adopted in 2012 when the current First Lady took office, features a pair of hands open to the sky, colored as the flag of the Dominican Republic. Above the open palms is a stylized family, showing the man in blue, the woman in red, and a small child between them that is half-blue and half-red. Below the hands, in black letters, is the name of the office and the country on four successive lines, one above the other:
DESPACHO
PRIMERA
DAMA
REPÚBLICA DOMINICANA
Of note, the capital U in "República Dominicana" shows an accent mark despite the apparent convention to the contrary in Spanish and other Romance languages.

The previous flag used by the Despacho Primera Dama is also white with the previous seal in the center. I assume that the seal and flag were both adopted in 2004 when the previous Presidential administration and First Lady took office. That seal has a green sphere in the center with three white, stylized people, represented as upward arrows pointing at white circles. Below the green sphere is an open book in shades of green. Two stylized hands are shown: a red one reaching up from the book on the hoist side of the green sphere, and a blue one reaching down toward the book on the fly side of the sphere. Surrounding the entire scene is the name of the office and the name of the country. At the top are a blue ribbon with white capital letters spelling "REPUBLICA," opposite a red ribbon with white capital letters spelling "DOMINICANA." Arched below the scene in green cursive script are the words "Despacho Primera Dama." Following on the note from above, the capital U in "Republica" has no accent mark in the 2004 version of the seal and flag.
