It is a fairly typical Portuguese municipal flag, with the coat of arms centred on a field gyronny (meaning city rank) of red and white..

The coat of arms is azure, a castle argent masoned sable with windows and doors gules, charged with Portugal ancien, by a fess wavy argent. Collar of the order of the Tower and Sword. Mural crown Argent with five visible towers (city rank) and white scroll reading in black upper case letters "CIDADE DE SANTARÉM".
Meaning:
The fess on the coat of arms stands for the Tagus river, and the castle for the Santarém castle, naturally.
