It is a typical Portuguese communal flag, with the coat of arms centred on a background quartered of yellow and red (town rank).

The arms are Argent, a hand proper, in dexter chief a towered tower Purpure masoned Or with port and windows Or, in sinister chief a bunch of grapes Gules leaved Vert. Mural crown Argent with four visible towers (town rank) and white scroll reading in black upper case letters "NUMÃO".
Meaning:
While "numão" has no evident meaning in morning Portuguese, "mão" means "hand" and the arms may thus be canting for that, as a pun or not. The official blazon calls for red grapes (and that's how this colour can be used on the flag), but Sérgio Horta uses purple.

Are you sure about the colour of the grapes? They are depicted rather Purpure; I guess real red grapes aren't all that red either.

Well, it is heraldic stylization. Portuguese subnational arms abound in grapes, but those are mostly Purpure, some times Argent; there are only a few instances of Or (and Vert?), but I can remember only another case of Gules that is so variantly depicted but blazoned Argent: Arruda dos Vinhos Municipality.
