In Presidente Hayes government website, www.gobernacionxv.gov.py, and especially at:
http://www.gobernacionxv.gov.py/museo.htm or at http://www.gobernacionxv.gov.py/contacto.htm it can be seen a green-white-green horizontally divided animated gif flying.
I could not find anywhere else on the site a presentation of this flag as
the departmental flag, but I don't see what else it could be, the way it is
displayed.

At: http://www.gobernacionxv.gov.py/museo.htm (bottom of the page),
a photo shows what is said to be "the first flag of the government" of the
department.
The picture is too small for me to say if this is a rectangular flag or a
gonfanon-shaped object, but it is white with green stripes from the upper
corners to the center where is the coat-of ams of the department.

It seems to be a rectangular flag haning from three points along the top
edge, the middle one a bit higher than the others (note the drape riples
in the central part of the flag).
I squashed the original image and attach the result as
