The coat of arms and flag of Poljčane are prescribed by Decision Odlok o grbu in zastavi Občine Poljčane , adopted by the Municipal Council on 24 March 2009, and published in the official Slovene gazette Uradni list Republike Slovenije 2009/28 on 10 April 2009.
The flag, in proportions 2:5, is divided white-green by a chevron with the municipal coat of arms in the white field, which is nearly square.
The flag is a commpromise between proposals #1 and #3 designed by Valt Jurečič.

The coat of arms of Poljčane, designed by Valt Jurečič, is "Argent in the base of a pile reversed vert a barullet wavy of the first and in chief two flowers purpure seeded or and vert."
The flower is a pasqueflower (Pulsatilla grandis), a protected flower growing in the municipality.

Valt Jurečič, who had already designed the municipal coat of arms, presented 15 proposals for the municipal flag.
The local commission selected three of them to be presented to the
Municipal Council for adoption, as explained in the minutes of the 13th Session of the Council, held on 18 March 2008. The materials for this session includes the report of the commission and drawings (part a & part b) by Valt.
The three final proposals were:

1. A green flag with, next to the hoist, a white panel, almost square, containing the entire coat of arms;
2. A similar flag but with the coat of arms of smaller size, set in the canton of the white field;

3. A green flag with the white panel ending as a triangle into the green field, with only the attribute of the coat of arms, the pasqueflower, blue, unlike on the coat of arms.
Each proposal was drawn in horizontal and vertical version, with the usual 2:5 proportions.
The votes of the Municipal Council were tied (7:7) between proposal 1 and 3, and the decision was left for the next session.
For the next, 14th session, held on 6 May 2008, Valt Jurečič, who attended all the commission sessions and all the relevant sessions of the Municipal Council as an expert, prepared another proposal, similar to aforementioned proposal 1., but without a border around the coat of arms. After the discussion, the councillors voted 8 for the proposal with the coat of arms and 4 for the proposal with the flower (and none for the new variant without the border). The minutes of the session conclude therefore that the decided design is proposal I numbered 1 above.
